Join a leading healthcare provider as a Care Partner, supporting Support at Home and NDIS clients across Sydney’s Eastern Suburbs.

Based in a modern metro office location, you will play a key role in coordinating high-quality care services, leading and supporting a team of carers, and ensuring clients receive personalised care that enables them to live independently at home.

• $120,000 - $130,000 + Superannuation
• Permanent full-time role, Monday to Friday
• Lead and support a team of 60+ carers while working alongside an experienced and supportive management team

Key Responsibilities:

• Complete initial client assessments and develop tailored care plans
• Coordinate and oversee Support at Home & NDIS clients
• Work collaboratively with Allied Health teams and care staff
• Build and maintain referral pathways and stakeholder relationships
• Contribute to the ongoing development and implementation of premium Home Care services
• Ensure high quality client outcomes and service delivery standards are maintained

About you:

• Current AHPRA Registration with experience as a Registered Nurse
• Previous experience managing NDIS & Support at Home clients
• Strong clinical skills and a compassionate approach to aged care
• A team player with excellent communication and leadership abilities
• Ability to problem-solve, take initiative, and adapt in a fast-paced environment
• Australian Drivers License
