Case Manager (Clinical) - Work in Regional South Australia
An established, community-focused aged care provider is seeking 3 compassionate and experienced Case Manager (Clinical) to join their team supporting clients across regional South Australia.

- Manage a caseload of clients with complex clinical and social needs
- Conduct comprehensive clinical assessments and develop care plans
- Implement, monitor, and evaluate interventions using evidence-based practice
- Coordinate services including assistive technology and home modifications (AT-HM)
- Monitor package budgets and ensure effective utilisation aligned to client goals
- Liaise with external health providers and stakeholders
- Respond to changes in client condition and escalate where required
- Maintain accurate and compliant documentation
- Bachelor’s degree in Nursing or relevant Allied Health discipline
- Minimum 2 years’ experience as a Registered Nurse or Allied Health Professional
- Experience within Home Care Packages (HCP) and/or CHSP (desirable)
- Experience managing care budgets and utilisation
