Community Case Manager

Your Responsibilities:
  • Build and maintain strong relationships with clients and their families.
  • Ensure quality and compliance by conducting regular visits to clients' homes.
  • Assess client needs, develop individualized care plans, and evaluate their effectiveness.
  • Collaborate with the Rostering team to ensure the right skill mix is provided for clients.
  • Develop and sustain connections with internal and external stakeholders to deliver a range of services.
What We’re Looking For:
  • Experience: Minimum of 1-2 years in the Home Care industry.
  • Qualification: Cert IV in Aged Care or equivalent qualification.
  • Skills: Proven ability to build strong community networks and relationships.
  • Knowledge: In-depth understanding of the Aged Care sector, including Home Care Package guidelines and service requirements.
