Community Case Manager

We are searching for a Case Manager with a Registered Nurse qualification who can utilise their clinical background in the care for this provider's Home Care Package (HCP) clients.

Your Responsibilities:

Build and maintain strong relationships with clients and their families.
Ensure quality and compliance by conducting regular visits to clients' homes.
Assess client needs, develop individualized care plans, and evaluate their effectiveness.
Collaborate with the Rostering team to ensure the right skill mix is provided for clients.
Develop and sustain connections with internal and external stakeholders to deliver a range of services.

What We’re Looking For:

Experience: Minimum of 1-2 years in the Home Care industry.
Qualification: Clinical qualification as either a Registered Nurse, Physio, or OT.
Skills: Proven ability to build strong community networks and relationships.
Knowledge: In-depth understanding of the Aged Care sector, including Home Care Package guidelines and service requirements.
