Seeking a Clinical Care Manager (relocate) to oversee all clinical operations at a Residential Aged Care Facility in regional VIC (2.5 hours fom Melbourne). You will directly report to the General Manager and overseeing the clinical care in the 100+ bed home.

- Oversee the daily clinical operations of the facility
- Support and lead the clinical team including coaching, mentoring, staff allocations and delegation, performance management and education
- Review clinical incidents, complaints and changes
- Lead compliance and accreditation preparations
- Manage person-centred care planning and evaluation for residents - including case conferencing with families and carers
- Prepare and conduct clinical governance audits
- Oversee medication management in accordance with resident needs and legislative requirements
- Current AHPRA as a Registered Nurse
- Previous working experience within aged care as a CCC/CM/CCM
- Proven track record in delivering quality clinical care
- Excellent clinical assessment skills and care planning capabilities
- Highly effective communication, leadership and relationship management skills
- An understanding of Resident Rights, Aged Care Accreditation Standards and Outcomes
