Are you a passionate and committed Nurse Practitioner looking to make a meaningful difference in Palliative Care? About the Role As a valued Nurse Practitioner, You’ll provide expert clinical assessments and implement advanced, patient-centred care plans while leading initiatives in evidence-based practice and contributing to clinical service planning. The role also involves actively supporting quality improvement initiatives and mentoring staff to strengthen clinical capability across the team. In addition, you will work closely with the Community Palliative Care Team to ensure coordinated care and improved patient outcomes across a variety of care settings.
NPs across our services will build strong, trust-based relationships, support continuity of care, and work collaboratively with Nurses, Doctors, Specialists, Elders, and families.
