Roving Care Manager
Seeking Registered nurses with excellent clinical skills and care management experience to assist with clinical improvement projects at Aged Care facilities across Northern Queensland.

Depending on location, applicants may be required to stay away from home on occasions while covering Care Manager leave.  Travel and accommodation costs will be covered by the organisation.

The Role
  • Supervise staff in respect of the quality of care provided to residents with a person centred approach. 
  • Lead by example and act as a role model to the staff. 
  • Work well in a multidisciplinary team and provide resident focused care ensuring to report any issues to the relevant parties.
  • Assist homes with assessment/care planning responsibilities and the implementation of improvement plans 
  • Provide clinical leadership to homes, in the delivery of care, monitoring of clinical systems and in addressing practice and process deficits.
  • Create appropriate learning, development and educational activities
Requirements 
  • Current AHPRA registration as an RN 
  • Previous Clinical Management experience 
  • Proven track record in continuous improvement projects
  • You have extensive experience with Memory Support and Palliative Care 
  • Strong understanding of Quality Management Systems and Residential Aged Care accreditation
  • Post graduate certificate/diploma in health-related discipline (Desirable) 
  • The ability to lead practice change through role modelling and education
  • High level of interpersonal and communication skills (written and verbal)
  • Demonstrated understanding of person-centred care and ability to work in a team environment
