Join a fully accredited, boutique aged care facility with an excellent reputation for quality care and community engagement. We’re seeking a passionate and experienced Care Manager to lead and support a clinical care team in delivering safe, high-standard, person-centred care.

About the Role
Reporting to the Facility Manager, the Care Manager will oversee the clinical operations of the facility, ensuring compliance with Aged Care Quality Standards and continuous improvement in care delivery. You’ll lead by example—supporting RNs and carers to deliver compassionate, consistent, and safe care to all residents.

Responsibilities: 
  • Oversee clinical admissions, care planning, and reassessments
  • Monitor documentation accuracy and timely progress note completion
  • Conduct and support care conferences
  • Supervise medication management and respond to clinical incidents
  • Support performance appraisals and manage RN/carer rosters
  • Guide and mentor care staff in best practice care delivery
  • Drive quality improvement and clinical governance
  • Ensure compliance with all relevant aged care legislation and standards
Requirements: 
  • Current AHPRA registration as a Registered Nurse
  • Minimum 5 years' experience in aged care, including 2+ years in a leadership role
  • Strong knowledge of the Aged Care Quality Standards and relevant legislation
  • High-level leadership, interpersonal, and organisational skills
  • Experience in managing and mentoring multidisciplinary care teams
  • Confident using electronic health record systems
  • A hands-on, supportive leadership style with a focus on resident outcomes
