This role allows you to take full ownership of your own clinical caseload, providing high-quality support to older Australians in their homes.
About the Role
- Permanent, full-time position – available now

- Monday to Friday – no weekends and minimal on-call

- Report to an experienced and supportive manager who values collaboration
- Current AHPRA registration as a Registered Nurse
- Home Care Package (HCP) experience is essential
- Commonwealth Home Support Programme (CHSP) experience is desirable
- Recent clinical experience in either community or residential aged care
- Proven case management experience within aged care, health, or disability sectors
- Strong computer literacy and documentation skills
- Excellent time management and organisational abilities
