- Ensure quality and compliance by conducting regular visits to clients' homes.
- Assess client needs, develop individualized care plans, and evaluate their effectiveness.
- Grow your client caseload through proactive business development
- Develop and sustain connections with internal and external stakeholders to deliver a range of services.
- Experience: Demonstrated background in a case management setting
- Qualifications: Certificate IV in Aged Care or an equivalent qualification (preferred).
- Skills: Proven ability to establish and maintain strong community networks and relationships.
- Knowledge: Solid understanding of the Aged Care sector, including Support at Home, Home Care Package guidelines, and relevant service requirements.
