As Care Manager, you'll lead clinical care delivery and service outcomes at a close-knit residential aged care home, driving continuous improvement in resident care. You'll support and coach a committed care team to deliver quality, person-centred care, and oversee clinical governance and care planning — backed by a dedicated onsite Clinical Manager/Educator and a central Clinical Governance, Quality and Education team.
Main duties include:
- Leading day-to-day clinical care delivery and service outcomes
- Driving continuous improvement in resident care and clinical outcomes
- Supporting and coaching a committed team to deliver quality, person-centred care
- Overseeing clinical governance, care planning and compliance, with central team support
- Building positive relationships with residents, families, staff and the local community
- Registered Nursing qualifications with current AHPRA registration (essential), with extensive relevant aged care industry experience
- Demonstrated knowledge of aged care regulatory requirements and active participation in the Accreditation system
- Proven experience with aged care funding streams, including AN-ACC and associated documentation
- Sound understanding of staff performance, development and rostering
- Demonstrated commitment to EEO, Infection Control, Continuous Improvement and WH&S principles
- Strong report writing, communication and interpersonal skills
- Strong decision-making, problem-solving and conflict-management skills
