We are currently working with a highly reputable provider of Home and Community Care services searching for an experienced Case Manager to ensure the highest quality of services provided to clients are maintained.
The role:
Opportunity to make a real difference by collaborating with clients in a community setting, providing expert Case Management services!
- Sitting in a supportive and close-knit team of Care Managers
- Manage your own case load of up to 50+ clients
- Hybrid working arrangement. This role involves a mix of WFH to work on care plans and home visits with clients
- Reporting to Senior Care Manager and Operations Manager
- Up to $84k + super (based on experience)
- Flexible work arrangements - Only required in the office one a month!
- All travel costs covered and company mobile will be provided

- Experience: Minimum of 1-2 years in a similar role within the Home Care industry.
- Qualification: Clinical qualification as either RN, Physio, or OT is highly desired.
- Skills: Proven ability to build strong community networks and relationships.
- Knowledge: In-depth understanding of the Aged Care sector, including Home Care Package guidelines and service requirements.
- Transport: Own vehicle and valid driving license is essential for this role
