We are seeking a passionate and experienced Care Manager to lead the clinical operations at a well-regarded, 45+ bed Residential Aged Care Facility in rural Western Australia. This is a fantastic opportunity to step into a leadership role within a supportive, values-driven organisation that truly cares about its residents and staff.
About the Role: As the Care Manager, you’ll be responsible for overseeing the delivery of high-quality clinical care to residents, supporting and supervising a team of Registered Nurses and care staff. You’ll ensure clinical best practices are followed and that the team is aligned with Aged Care Quality Standards and internal policies.
Key responsibilities include:
- Overseeing admissions, care planning, and resident re-assessments
- Ensuring adherence to clinical governance and accurate documentation
- Supporting medication management and incident reporting
- Leading scheduled care conferences and clinical team development
- Providing hands-on clinical guidance and direction
- Conducting performance reviews and supporting staff development
- Contributing to continuous improvement initiatives
- Managing rosters, workload distribution, and team performance
- AHPRA Registered Nurse
- Minimum 5 years’ experience in aged care, including 2+ years in a leadership role
- Strong knowledge of the Aged Care Quality Standards and aged care legislation
- Hands-on leadership style with excellent interpersonal and organisational skills
- Confident using electronic health record systems
- A values-driven approach and commitment to quality resident care
