Clinical AN-ACC Support Officer – Aged Care
We are seeking a Full-time Clinical AN-ACC Support Officer to optimize care planning and funding for this reputable aged care provider

Benefits
- Permanent Full-time position
- $58 per hour (approx. $120,000 + super + salary packaging)
- Work with a reputable aged care home, well running with a supportive facility manager
- Monday to Friday position with a fixed hour work day
- Work in a state of the art facility offering residents a abundance of features and daily activities
- Will consider Senior RNs and CMs with AN-ACC experience
- 4 star compliance, fully accredited facility
- Conduct and oversee AN-ACC assessments for residents
- Care planning, admissions and AN-ACC assessments
- Working closely with the facility manager and clinical care coordinator
- Ensure accurate, timely, and compliant clinical documentation
- Complete HELF checks (on admission, annually and when required.
- Support wound care, pressure care, continence, nutrition, and mobility assessments
- AHPRA Registered Nurse
- Solid knowledge and experience in AN-ACC assessments
- eCase and BESTMED experience is highly desirable
- Experience in dementia support is highly desirable
- Police Check and Driver's License
- Full working rights
