Seeking an experienced and passionate Clinical Care Coordinator to join the leadership team at a medium-sized residential aged care facility which is 25 minutes from Melbourne CBD.
As Clinical Care Coordinator you will lead, inspire and mentor Registered Nurses and care staff while ensuring clinical care delivery is of the highest standard and compliant with ACQS. This is a fantastic opportunity to join a leading Aged Care provider and support positive change in the organisation.
About the Role:
The Clinical Care Coordinator will provide guidance, consultation & support to the onsite clinical team. They require an experienced professional to manage the clinical services on site, alongside an insightful leadership team.
- Report to the Director of Care
- Monday to Friday 7am - 3pm
- Manage care planning, compliance, and audit processes
- Maintain thorough documentation, reporting and strong understanding of SIRS
- Educate and lead staff on the implementation of incoming reforms and drive continuous improvement
- Supported by executive leadership, along with quality and AN-ACC Support from head office

- Structure: Owner/Director, Operations Manager, Director of Care Services, Clinical Care Coordinator, RN's, EN's PCA's, Lifestyle and Rostering Coordinator

Requirements:
- AHPRA Registration as a DIV 1 Registered Nurse
- 3-5 years' experience as Clinical Care Coordinator
- Previous experience in a leadership within aged care
- Proven understanding of AN-ACC funding, Care Planning quality indicators and SIRS
- Strong knowledge of aged care standards and incoming reforms
- Strong interpersonal and communication skills
