This position is responsible for developing personalized care plans and building close relationships with Support at Home clients and their families. You will work with a local client base with regular in person visits. This is an exciting opportunity to deliver person centered care and develop key relationships in the local community.

- At least 1 year experience in Aged Care Case Management.
- Highly efficient and organized with proven time management skills.
- Cert III in Aged Care.
- Home Care Package/ Support at Home experience is a must.
- Proficient in using the Microsoft Suite with strong skills in Excel, Word & Outlook.
- Strong written and verbal communication skills.
- Must hold a valid Australian Driver’s License and be able to commit to local travel.
