Roving Care Manager
Are you an experienced Care Manager looking for a role that offers variety, leadership and the opportunity to make a meaningful difference?

Join a leading Australian aged care organisation as a Roving Care Manager, providing clinical leadership and support across a portfolio of care homes throughout Victoria and New South Wales.

This is an excellent opportunity for an experienced aged care professional who enjoys working in different environments, supporting teams through change and delivering consistently high standards of resident care.

  • 5 years' care management experience highly desirable
Key Responsibilities:
  • Oversee daily clinical operations to ensure residents receive high-quality care.
  • Lead, coach and mentor clinical teams, including staff allocation, delegation, education and performance management.
  • Review clinical incidents, complaints and changes, identifying solutions and opportunities to reduce risk.
  • Lead compliance with AN-ACC assessments.
  • Oversee person-centred care planning and evaluation, including case conferencing with families and carers.
  • Foster a positive, collaborative and accountable team culture.
  • Prepare and conduct clinical governance audits.
  • Identify opportunities for continuous clinical improvement and implement innovative solutions.
  • Oversee medication management in accordance with resident needs and legislative requirements.
  • Collaborate with allied health professionals, medical practitioners and other service providers to optimise resident outcomes.
Key Requirements:
  • Bachelor of Nursing with current AHPRA registration - essential.
  • Postgraduate qualification in aged care - highly regarded.
  • Previous Care Manager experience, with approximately 5 years desirable.
  • Proven experience delivering high-quality clinical care within aged care or an acute setting.
  • Strong leadership, communication and interpersonal skills.
  • Excellent clinical assessment, triage and care planning capabilities.
  • Strong understanding of clinical governance, risk management and continuous improvement.
  • Demonstrated ability to foster a positive reporting culture and conduct effective investigations.
  • Understanding relevant aged care accreditation standards and re-registration requirements.
  • A genuine passion for person-centred care and improving outcomes for residents.
  • Flexibility and willingness to undertake travel assignments across VIC and NSW.
