Roving Care Manager | Aged Care | VIC & NSW Portfolio
One of Australia's largest regional aged care and dementia providers is seeking a Roving Care Manager to step into a genuinely fulfilling and varied role, working across a portfolio of homes in VIC and NSW. Travel assignments range from 1 week to 3 months, with all travel expenses, accommodation and a food allowance covered — plus the ability to fly home on weekends for longer assignments.

The Opportunity / Main Duties
  • Oversee daily clinical operations of the care home, ensuring residents receive the best care available
  • Support and lead the clinical team — coaching, mentoring, staff allocation, delegation and performance management
  • Review clinical incidents, complaints and changes, providing solutions to reduce risk
  • Lead compliance with AN-ACC assessments
  • Manage person-centred care planning and evaluation, including case conferencing with families and carers
  • Prepare and conduct clinical governance audits
  • Oversee medication management in line with resident needs and legislative requirements
  • Liaise with allied health, medical practitioners and other services to support optimal resident care
What We're Looking For
  • Bachelor of Nursing with current AHPRA registration (essential)
  • Postgraduate qualification in aged care highly regarded
  • 5 years' care management experience desirable
  • Strong leadership, communication and interpersonal skills, with a genuine person-centred care mindset
  • Proven track record delivering quality clinical care within aged care or acute settings
  • Excellent clinical assessment, triaging and care planning capabilities
  • Understanding of Resident Rights, Aged Care Accreditation Standards and Outcomes
  • Open to candidates based in either VIC or NSW, with flexibility for travel assignments
