Are you an experienced Senior Case Partner with a strong background in Home Care Package management and team leadership? We are looking for a confident, client-focused professional to manage HCP clients while driving business growth and mentoring your team.

- Lead and manage client relationships, including referrals, networking, and business development initiatives.
- Ensure the delivery of high-quality, person-centered care for all clients.
- Conduct thorough client assessments, care planning, and regular reviews in line with HCP guidelines.
- Oversee daily client care and maintain accurate, detailed documentation.
- Collaborate closely with the Care Manager and Regional Manager to support operational excellence.
- Contribute to business growth through active input on sales strategies and service development.
- 3-4 years experience in Case Management within the home and community care sector
- Relevant diploma or degree in Community Services, Health Sector Management, Aged Care, or Nursing
- Proven business development experience with strong sales acumen
- Excellent communication, networking, and relationship-building skills within the community
- In-depth knowledge of the Aged Care sector, including HCP guidelines and service requirements
