Senior Clinical Care Partner - Community Aged Care - Brisbane Northside

I’m partnering with a well-established aged care provider to recruit an experienced Senior Clinical Care Partner for a senior leadership role within their community aged care services.

This is an excellent opportunity for an experienced Registered Nurse who enjoys leading people while remaining close to clinical practice, quality and client outcomes.

The Role You’ll provide clinical leadership and oversight across a community aged care program, supporting a multidisciplinary team to deliver safe, coordinated and person-centred care.

Key responsibilities will include:
  • Lead and mentor nursing and care teams, building capability and consistent clinical practice.
  • Oversee complex client care, clinical assessments, referrals and goal-based care planning.
  • Drive clinical governance, quality and compliance in line with Support at Home requirements and the Strengthened Aged Care Quality Standards.
  • Coordinate multidisciplinary services across nursing, allied health and community care.
  • Oversee client funding and resources, ensuring budgets are managed appropriately and services align with assessed needs.
Essentials:
  • Current AHPRA registration as a Registered Nurse
  • Strong clinical leadership and people-management experience
  • Sound knowledge of the Support at Home Program
  • Knowledge of the Strengthened Aged Care Quality Standards
  • Strong assessment, care planning and case-management capability
  • Community or aged care experience highly regarded
  • Current driver’s licence and ability to travel within the community
