Senior Project Lead – Aged Care & General Practice - 12 month contract

We’re seeking an experienced and forward thinking Aged Care Professional to deliver a groundbreaking initiative designed to strengthen collaboration between General Practice and Residential Aged Care.

In this role you’ll take the reins of a sector leading program that builds clinical governance, improves communication, and enhances health outcomes for residents across multiple Aged Care Homes in the Shepparton region.

This position is perfect for someone who wants to move beyond the floor and make a system wide impact — shaping how aged care and primary health
work together to deliver truly coordinated, person-centred care.

You’ll partner closely with GPs, Practice Nurses, Aged Care Providers, and Allied Health teams to implement a best practice Nurse Coordination Model that supports continuity of care, reduces avoidable hospital admissions, and strengthens workforce capability.

About you
  • Registered Nurse (AHPRA) with 5+ years’ experience in aged care
  • Proven leadership in clinical coordination, governance or service improvement
  • Excellent relationship-building and stakeholder engagement skills
  • Strong understanding of aged care funding models (AN-ACC), accreditation, and compliance frameworks
  • Postgraduate qualifications in Aged Care, Dementia, Palliative or Mental Health Nursing highly regarded
